Respirator Use Clearance Statement

For 40-Hour HAZWOPER and 24-hour Emergency Response Students Only:

I understand that I must be cleared for respirator use by a health care professional in order to participate fully in the "Hands-On/Dress-Out" component of these classes.

Payment and Class Locations
You will receive a confirmation email within 3 business days containing information about payment options and class locations.

Refunds and Cancellations
Full refunds will be made when notice of cancellation is received at least ten (10) days prior to the start of class. Cancellations made within the ten (10) day period immediately preceding a class will be assessed a $75 administrative fee. Failure to show (without prior notice) will forfeit full tuition. Substitutions are accepted at anytime. AMEC Environment & Infrastructure reserves the right to cancel any class due to insufficient enrollment, in which case, a complete refund will be issued. AMEC Environment & Infrastructure will not be responsible for any travel-related expenses. For additional information, please contact our Training Coordinator.
